Forms: Intake Paperwork

Please print out and complete the Client Registration form and only the last page of the Informed Consent document and bring them with you to your appointment.  The informed consent is a long document, so we don’t ask you to print it all out, as you can have it for reference in your computer. The personal disclosure statements are for your information, and required by our law.  We will happily answer any questions you might have when you come in.
